Document Requirements

Bali Visas application form.
Child’s Indonesian & Foreign Passport.
Child’s Identification Card (KIA).
Scan of the identity and signature pages of the parent’s passport (no photos, must be a color scan).
Child’s birth certificate or Akta Lahir.
Indonesian ID Card (KTP) of parent(s).
Family certificate (Kartu Keluarga).
Scan of most recent visa sticker/stamp in passport of foreign parent.
Marriage certificate of parents.

Renewal

Renewal needs to be processed within 2 - 3 weeks before Affidavit expires.
All previously required documents must be resubmitted as well as the Affidavit card itself.
